How to Register For Patient Access

You must register your details for this service before you can create an account. It’s quick and easy to register – simply follow the three steps below.

Remember, when using Patient Access all your personal information is safe and secure.

Register in 3 easy steps

Step 1

Ask at reception for a registration form or print and complete the form below. This provides all the information you need to register to use the Patient Access internet facilities. We need to verify your identity at this stage so please bring suitable photo-id (such as a passport or driving licence) and evidence of your home address (such as a bank statement or utility bill).

Step 2

Click on the link on the relevant page to visit the Patient Access website and create your account.

Step 3

Enter your personal registration details exactly as they appear on the registration form. If the information does not match, your registration will not be accepted.

PLEASE keep your password safely. If you forget your password you will need to contact us and we will have to send you new PIN codes and you will have to register all over again.
