GP referrals
To request a GP referral:

Provide as much information as you can as this will help us to process your request.
Tracking a referral
To track an existing referral:
- In the first instance, contact the hospital or service you have been referred to. If you do not have these details, phone us on Grosvenor Medical Centre 01270 256348 or Gresty Brook Surgery 01270 650012 and we will try and provide these details to you.
