The practice nurses provide a wide range of nursing services including regular reviews and follow up for patients with long term conditions:

• Diabetes
• Heart Disease
• Asthma – If you have been advised by the surgery to submit an annual review of your asthma symptoms please use our Asthma Review form
• Chronic Obstructive Pulmonary Disease (smoking related respiratory problems) – If you have been advised by the surgery to submit a COPD assessment please use our COPD Assessment form

Other services provided are:

• Cervical Screening (Smear tests)
• Childhood Vaccinations
• Adult Vaccinations including Flu and Pneumonia
• Cardiovascular Risk Assessments (Cholesterol/BP checks)
• Wound Care, including Leg Ulcer management.
• Blood Pressure Checks (including 7 day home monitoring– If you have been advised by the surgery to submit your blood pressure readings on a regular basis please use our Blood Pressure Review form
• ECGs
• Doppler Assessments (checks circulation in legs)
• Telephone Advice (for minor health issues)
• Travel Health Advice and vaccinations If you are travelling abroad and need to book travel vaccinations, please use our Travel Risk Assessment form
